While Change Healthcare cannot confirm exactly what data has been affected for each impacted individual, information that may have been impacted includes:
- Contact information (e.g., name, address, date of birth, phone number, email)
- Health insurance information (e.g., member/group ID numbers, Medicaid-Medicare-government payor ID numbers)
- Health information (e.g., medical record numbers, diagnoses, medicines)
- Billing claims and payment information
- Other personal information (e.g., Social Security numbers, driver’s licenses)
How do I get a copy of my credit report?

To order your free annual credit report, visit Annual Credit Report, call toll-free at 877-322-8228, or complete the Annual Credit Report Request Form on the FTC’s website.
How can I enroll in free credit monitoring with IDX?

If you believe your information may have been impacted, you can enroll in two years of complimentary credit monitoring and identity protection services provided by Change Healthcare. Sign up by clicking Enroll now or by calling 1-888-846-4705.
How can I check on my health care information?

Regularly monitor your explanation of benefits statements from your health plan and health care providers as well as financial documents for any unfamiliar activity.
When will I know if my data was impacted and how will I be notified?
- The data review is in its late stages. Change Healthcare has begun mailing written notices via U.S. mail
to affected people identified in the ongoing data review for whom Change Healthcare has a sufficient
address. Please note, Change Healthcare may not have sufficient addresses for all affected individuals.
The mailing process began on July 29, and will continue on a rolling basis.
