A complete step by step walk through of the Arizona Electronic Fingerprint Process

This is a two part process

The Division of Insurance (DOI) issues many licenses that may require the need for a Background Check. DOI does not accept AZ DPS Fingerprint Clearance Cards.

This process is a multi step process requiring individuals to establish accounts for both the AZDPS Public Services Portal and Fieldprint websites.  Located below is a summarization of the steps needed, pertinent fingerprint codes, and reference notes of information that should be retained throughout the licensing application process.

Your fingerprints and Your Rights to Your Criminal History Record Fingerprints that you provide in conjunction with a license application will be submitted to and checked against Arizona and FBI criminal history databases.

You have the right to review, and will have up to 60 days, to challenge your own criminal history records to correct or complete the record (or decline to do so) before your license is officially denied based on information in the criminal history record. Privacy Act Statement 

Arizona Criminal History Record:
To obtain a copy of your Arizona criminal history record in order to review/update/correct the record, you can contact the Arizona Department of Public Safety (ADPS) Criminal History Records Unit at (602) 223-2222. Information concerning the review and challenge process is available on the ADPS website at https://www.azdps.gov/services/public/records/criminal.

Federal Identity History Summary Checks: The procedures for changing, correcting or updating your FBI criminal history record are set forth in Code of Federal Regulations (CFR) Title 28, Chapter I, Part 16, Subpart C (§§ 16.30 through 16.34). Information on how to review and challenge an FBI criminal history record is available from the FBI website at https://www.fbi.gov/file-repository/challenge-brochure.pdf/view.

Arizona Department of Public Safety Public Service Portal

Below is a link to the Arizona Department of Public Safety Public Service Portal.  Use this portal begin to complete the background check of the complete fingerprint clearance card process.

Upon accessing the portal click on icon "Background Check".

Log in with an existing account or click "It's easy to create one " to establish a new account for an "Individual"

Enter in the appropriate fingerprint code

  a. Bail Bond and Bail Recovery Agent applicants - BRA-056.A

  b. Navigator Applicants- NDI-053.A

  c. Certified Application Counselor Applicants - DIC-0564.A

  d. All other insurance professional applicants - INS-055.A

Some fields will vary depending on the fingerprint code entered.  Please read all prompts carefully.

  *Note* - Under the prompt "Agency Information" please either select applicant or employee.

 **Please be aware a fee of $22.44 to be paid via Credit Card must be completed before the application can be processed**

IMPORTANT

After payment has been made you will receive a "Reference Number" that will begin with the letter A followed by numeric digits.  Retain this number as it will be needed two additional times in your licensing application process.

Fieldprint

Below is a link to the Fieldprint Arizona site.  Use this portal begin to schedule an appointment to complete the livescan portion of the process.

To begin this process click on the "Schedule Appointment" on the home page.

For new users to this site select - "Sign Up" and follow the prompts.  For Existing User please select "Log In". 

IMPORTANT

All users need to scroll down and select "Don't have a Fieldprint Code" as well as "Continue with this Reason"

Enter in the "Reference Number" that will begin with the letter A followed by numeric digits that you were supplied from AZ DPS as well as your vital information requested.  

Follow the prompts to schedule an appointment. 

 **Please be aware a fee of $8.25 will need to be paid in order to confirm the scheduled appointment**

A confirmation will be emailed to the email address supplied during registration.  

 

Additional Information

The Reference number supplied by AZ DPS during the background check will be needed to complete your NIPR application.  Please retain this information.
