Certain policy forms and advertisements may be exempt from Arizona Revised Statutes, Title 20 filing requirements. Please review the Department's exemption order listing all exempt forms and advertisements.

The Department’s exemption order is issued pursuant to ARS §§ 20-1110(F) and 20-1691.08(F) and filed under Docket No. 22A-051-INS (the “Exemption Order”). The Exemption Order directly supersedes the Department’s previous exemption order filed under Docket No. 15A-005-INS. The Exemption Order is
available on the Department’s website at https://difi.az.gov/insurers/life-health-forms-rates-compliance/lifehealth-form-filing and has three exhibits (A, B, and C) that list the forms and advertisements that are exempt from filing.

Please note that while you may not need to file certain forms and advertisements pursuant to the Exemption Order, all products in use in Arizona must comply with all other applicable laws.
